Injections of various foreign materials have been around for as long as there have been sharp hollow tubes and foreign materials.  Silicone and other substances have been injected into the face and almost every other body part you could imagine.  The most common foreign body injections at this time are performed for buttock, hip, lip, and face augmentation.

These are certainly not recommended by plastic surgeons, and these injections are frequently performed by individuals without appropriate training using less than sterile material that is not FDA approved for injection in the first place. There are numerous complications with these injections including skin loss, pulmonary emboli, muscle death, patient death, massive infection leading to death, etc.  To be perfectly clear, butt augmentation should occur through only two routes: 1) transfer of fat referred to as a Brazilian buttlift or 2) butt implant surgery.

Patients frequently report ongoing inflammation and chronic pain syndromes that can result in moving pains throughout the body from the legs down into the ankles and into the abdomen as well.  Some of these chemicals that spread in this nature are liquid and are very difficult to completely remove.  These include hydrogel materials, biopolymers, and silicone along with other injected oils.

Dr. Kenneth Hughes removes these types of materials on a regular basis, and he attempts to do so through very small incisions placed and hidden appropriately.  For larger removals, larger incisions will have to be made.  To be clear, liposuction WILL NOT remove these oils in any meaningful way.  The scar tissue and inflammation that develops around the silicone or hydrogel will prevent the liposuction cannula from being able to access these small pockets of oil.  For larger removals, there will be a great amount of skin that will need to removed as well, particularly if the skin is very diseased as well.

The easier removals include solid substances like PMMA. These are discrete materials that can be removed without destroying the innate tissue. This makes for a much easier reconstruction when it comes to fixing the defects caused by the removal of the substances. Unfortunately, most of the illegal materials injected are liquid viscous oil that is not only impossible to completely remove, but the tissue damage wrought by these materials may be overwhelming for the individual, leading to fulminant infection and death.

If they are not a source of ongoing infection, they can be very damaging to the skin, fat, and muscle and the blood supply to these tissues.  Skin that is discolored, purple, or black is severely damaged and may not heal after surgery or may require additional surgeries just for the removal of this dead or compromised tissue.

These silicone or hydrogel removal surgeries are not easy to perform and come with a litany of complications based upon how poor the tissue of the patient is at the time of surgery and how much silicone has been injected as well as where it has been injected.  Silicone injected into the muscle will not be removed due to the incapacitating effects of removing muscle tissue and its consequences in regard to posture, ambulation, and overall function in addition to greater risk for silicone emboli in this more vascular bed.

These removal surgeries may require 1, 2 or more surgeries to get as much removed as possible.  In addition, the list of complications includes tissue loss or death, incision separation, wound breakdown or poor scarring, deformity, multiple surgeries, multiple hospitalizations, fluid collections, infection, bleeding, multiple reconstructions, and others.

These surgeries are not taken lightly by Dr. Hughes. Dr. Hughes is not a salesman and he will not sell you anything.  However, if the condition of the tissue and silicone is causing a great deal of inflammation, swelling, itching, pain, or other constitutional symptoms, these issues will NOT improve over time without surgical intervention.

Dr. Kenneth Hughes received a phone call from a former Brazilian buttlift patient who was distressed that another doctor had ordered an MRI and had removed some tissue for a specimen sent to a lab. Apparently, the other surgeon was unsure if something other than fat had been injected into this patient.

The patient was very emotional, and Dr. Kenneth Hughes was absolutely incensed to hear that some other surgeon would think that a board-certified plastic surgeon who had performed more than 4000 Brazilian buttlifts (or fat transfers to the buttocks) would have transferred some strange material into a patient’s buttocks. To be clear, the only 2 suitable methods for buttock augmentation are fat transfer to the buttocks and solid silicone gluteal implants (buttock implants).

Quite to the contrary, a significant portion of Dr. Kenneth Hughes’s practice is removing foreign material injected into patients by non-plastic surgeons and non-doctors sometimes in hotel rooms or garages. 2) The patient wanted to make the area appear smoother and make the buttocks appear shorter and rounder without a lot of loose, hanging skin. Dr. Hughes was able to improve all of these aspects at one surgery with the inferior buttock lift incision.

The inferior butt lift surgery is a surgery that very few surgeons perform or perform in a certain way. The goal of the procedure is not to flatten the buttock or to reduce the buttock/leg definition, but rather it is to tighten the skin of the posterior thigh and improve the buttock shape as well as the sagging or droopiness of the buttocks.
